Duties include but are not limited to:
Provides administrative, program, and technical support in the administration of the Primary Care Management Module (PCMM) web application within Ambulatory Care, and has principal responsibilities for the following:
Maintains electronic data for Ambulatory Care providers related to Primary Care panel assignments for each site within the North Texas Healthcare System. Develops and analyzes reports to evaluate current staffing utilization, project future trends, and presents findings to leadership. Develops, tracks, and provides reports to leadership indicating the quantity and type of PCP Change Requests. Prepare formal responses to National and VISN PCMM and Designated WH suspenses. Ensures reports are in compliance with national and facility regulations and sound practices are upheld with regards to PCMM management. Reports include but are not limited to trended provider change request data, panel capacity reports, and provider-specific patient assignment reports. Works with Medical Administration Service (MAS) PCMM Coordinator(s) to ensure new providers at each site within North Texas are entered into PCMM with accurate demographics including panel size, location, support team, etc. Ensures changes in demographics for existing providers are communicated to MAS (e.g. team or site changes, reduction or increase in expected panel capacity, etc). Verifies the eligibility of new North Texas Healthcare System patients reporting to Ambulatory Care, and coordinates with Nursing and MAS to ensure they acquires a timely appointment. Utilizes PCMM to accomplish assignments and reassignments to primary care panels for patients presenting to the Ambulatory Care office and at the request of Ambulatory Care leadership. Oversees and tracks the process of mass notification to Veterans of PCP changes at each Outpatient clinic and CBOC within our Healthcare System. Supports the peer review program in that he/she will coordinate the distribution of documents pertaining to this program. Performs extensive research to compile pertinent data for monthly reports submitted to Peer Review committee. Primary frontline personnel responsible for deescalating agitated Veterans who report to Ambulatory Care. Requires triaging their concern, collaborating with the appropriate PACT member (Physician, Nurse, or Clerk) to address the concern, or presenting the case to Ambulatory Care leadership, the leadership of the appropriate service, the Patient Advocates office, or to the Directors office. Assists Veterans and their families, staff, employees, service officers, local, state, and federal officials or any individual requesting assistance from Ambulatory Care. Receives and screens incoming calls to the Ambulatory Care office; provides warm hand offs to appropriate staff, provides information to the callers when deemed appropriate on behalf of Ambulatory Care administrative staff. Educates applicable Ambulatory Care personnel monthly at each site within North Texas on TB requirements, follow-up with personnel directly to increase compliance, reports to Ambulatory Care leadership personnel in risk of becoming noncompliant. Uses an electronic software program to create and adjust clinic informational cards for PACTs to disseminate to Veterans. Requisitions required office supplies, materials, and services. Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-06 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. Experience:
You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to:
Performs extensive research to compile pertinent data for monthly reports. Uses word processing software to create, copy, edit, store, retrieve, and print forms, memos, and letters. Uses database or spreadsheet software to create, enter, revise, sort or calculate, and retrieve data for reports. Prepares and disseminates event information such as flyers, brochures, service web page, etc. Finds and reserves event locations. Prepares reports on events. Receiving and screening incoming calls, provides warm hand offs to appropriate staff or provides information to the callers when deemed appropriate. Setting up events, activities, and developing marketing strategies to promote, publicize and increase awareness and participation for programs and events. Reviewing form letters, instructional material, and marketing tools and techniques for accuracy. This position requires a qualified typist. A qualified typist can type 40 words per minute based on a 5 minute sample with three fewer errors. You will be rated on the following Competencies for this position:

The work is sedentary as the incumbent is required to sit comfortably to do work. However, frequent walking, standing, bending and carrying of lightweight items is required. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
